Home Health Services

Occupational Therapy
Occupational therapy promotes physical and mental health well-being through the engagement in everyday activities. Our therapists support people with, or at risk of, a range of physical, developmental, and mental health disorders.
Physical Therapy
Physical therapy can be an effective treatment for medical conditions or injuries resulting in pain or mobility issues. Our physical therapists can correct functional movement imbalances while teaching you techniques to prevent injury.
Speech Therapy
Our speech therapists treat patients with a wide-range of problems and disorders. The goals of speech therapy includes improving pronunciation, strengthening the muscles used in speech, and learning to speak correctly.
VitalStim Therapy
This is a non-traditional therapy to treat patients suffering from Dysphagia that uses a small current that passes through external electrodes on the neck to stimulate inactive swallowing muscles.

Lymphedema Therapy
Our therapists can help treat swelling caused by Lymphedema. Treatments may include compression bandages or stockings, massage, sequential pneumatic pumping, skin care, and rarely surgery to remove swollen tissue. *Not available in all locations
Skilled Nursing
Our primary goal is to allow you to age in place in your home. Our skilled nurses can help you get better, regain your independence, become more self-sufficient, and maintain your current level of function and slow decline.
Hospices Services
Hospice is a compassionate service focused on patients and their families by addressing their physical, spiritual, and emotional needs as part of end-of-life care. We work together to ensure our patients' quality of life and their wishes remain at the forefront of care.
Physical, Occupational & Speech Therapy
Our team of therapists can work with patients to improve their strength, balance, mobility, and fine motor skills to help them perform their daily living activities.
Social Services
Our social workers help meet the emotional, social, and spiritual needs of the patient and their family by connecting them to counselors, bereavement support specialists, and other community outreach services.
Chaplain Services
Our chaplains provide spiritual support and guidance to patients and their families, regardless of religious affiliation, as they navigate the hospice journey.
Skilled Nursing
Our nurses oversee the patient's care while educating the patient and their family. They coordinate with the patient's physician and hospice medical director to ensure that all of the patient's needs are being met.
Bereavement Counseling
Our bereavement specialists provide support to patients and families for up to 13 months after a patient's death. We also host monthly bereavement group meetings to help serve the needs of our community.
Pharmacy Services
Pain management and symptom control is an important part of a patient's care plan, which is why we have 24/7 access to medication through our pharmacy services.
Home Health Aides
Our home health aides help take some of the burden off patients and their families by assisting patients with daily activities, providing emotional support, and even helping with light housekeeping tasks.
Volunteer Services
Our volunteers are there to help patients and their caregivers by offering companionship and respite care. If your family caregiver needs some time to rest or recharge, we can provide short-term relief.
